Tennessee 1915(c) Waiver services

Supported Living (ID/DD)

Comprehensive, 24/7 support in a safe, stable home environment.

Supported Living is designed for individuals who need ongoing assistance with daily living, safety, and community participation. Our trained staff provide continuous support while encouraging independence and personal growth.

What Supported Living Includes:

  • Assistance with daily living skills
  • Medication assistance and health-related support
  • Meal preparation and nutrition guidance
  • Personal hygiene and grooming support
  • Household management and safety monitoring
  • Transportation to appointments and activities
  • Community engagement and social participation
  • Goal-based skill development

Who This Service Is For:

Adults who require consistent support to live safely and independently in their home.

Semi-Independent Living (ID/DD)

Flexible support for individuals who can live independently with occasional assistance.

Semi-Independent Living provides scheduled support for individuals who do not require 24/7 supervision but benefit from regular guidance, skill building, and check-ins.

What Semi-Independent Living Includes:

  • Assistance with budgeting and money management
  • Support with household tasks and organization
  • Help with scheduling and attending appointments
  • Skill building for independence
  • Safety checks and wellness visits
  • Community participation and social support

Who This Service Is For:

Adults who live independently but need periodic support to maintain stability, safety, and personal goals.

Adult Habilitation Day – Community-Based (ID/DD)

Skill building, community engagement, and meaningful daytime activities.

Our Community-Based Adult Habilitation program helps individuals build essential life skills while participating in meaningful activities throughout the community.
