Getting Started Is Simple

Ebenezer Cottage Care accepts referrals for adults (18+) receiving services under the Tennessee 1915(c) Waiver or ECF CHOICES. Whether you are a Support Coordinator, family member, or individual seeking services, our team is ready to help you begin the process.

We make referrals easy, fast, and communication-focused.

Step by Step

How to Request Services

  1. Step 1: Contact Us Directly

    Support Coordinators and families may reach out using any of the following:

  2. Step 2: Provide Basic Referral Information

    Your coordinator or family representative will share:

    • Individual’s name
    • Date of birth
    • Current service needs
    • County of residence
    • MCO: BlueCare Tennessee, UnitedHealthcare Community Plan, or Wellpoint (formerly Amerigroup)
    • Contact information for the coordinator or family
    • Any immediate safety or support concerns

    This helps us determine the best service match.

  3. Step 3: Review of Service Eligibility

    Our team will review:

    • The individual’s current 1915(c) Waiver or ECF CHOICES enrollment
    • Requested service type: Supported Living, Semi-Independent Living, Adult Habilitation Day, Community Living Supports, Personal Assistance, Community Participation Supports, or Transportation
    • Service availability in their county
    • Staffing and placement options

    We communicate clearly with the coordinator throughout this step.

  4. Step 4: Meet-and-Greet / Introduction

    We schedule a meet-and-greet with:

    • The individual
    • Their family or guardian
    • Their Support Coordinator
    • Our agency representative

    This meeting helps everyone understand expectations, goals, and support needs.

  5. Step 5: Acceptance & Service Planning

    Once accepted:

    • We notify the coordinator
    • Begin person-centered planning
    • Review goals, preferences, and support needs
    • Coordinate with the MCO for authorization
    • Prepare the home or program environment
  6. Step 6: Service Start

    After authorization is received:

    • Services begin
    • Staff are assigned
    • Daily supports and documentation start immediately
    • Coordinators receive updates as needed

    We ensure a smooth, safe, and supportive transition.
